All parents and / or legal guidance acknowledge that he/she is 18 years or older.

1. Participate may also mean Parents, legal Guidance, Player, Coaches, Officiator and Volunteer
2. Participate must complete an application before the participate become eligible to play or workout in practice.
3. Participate acknowledge the use of players name (s) for promotion of the company and / or affiliates is based on a lifetime agreement.
4. Insurance – We understand that excess medical insurance is provided by Visionary Sports Association. If an athlete has other medical coverage, that coverage will be applied first. If an athlete has no other coverage, the provided policies become primary subject to terms and conditions of each policy. There is a deductible fee, and claims must be submitted within thirty (30) days of an injury or accident. We understand that the Visionary Sports policy applies to Visionary Sports and / or affiliate authorized events.
5. Release – We recognize that the possibility of physical injury associated with sports. We release Visionary Sports Association and their affiliates, officers, directors, coaches, employees, agents, volunteers, sponsors, participating athletes, other parents, representatives and successors, and the school, school districts, religious organizations and other owners of the facilities where practices, games maybe played and their employees (all of the foregoing are collectively referred to herein as the Visionary Sports Association), from any and all liabilities, claims, damages and expenses, whether known or unknown, sustained by the Player or the undersigned which in any way arise out of, or are connected with or relate to the participation by the Player in any activity (including without limitations transportation to and from activities) organized by or in any way associated with Visionary Sports Association.
6. Indemnity – We hereby indemnify Visionary Sports Association from and hold each of the harmless against any and all losses, liabilities, claims, damages and expenses which any of them may suffer or incur arising out of or resulting from the Player's participation in any activities of Visionary Sports Association or the breach by the undersigned, by the Player or by any member of the family of the undersigned or the Player, as the case may be, of the Rules and Regulations or this agreement.
